    BUSINESS NAME:
    Los Santos County Clinic
    TYPE OF BUSINESS:
    • [] Ambulance Service
      [x] Medical Clinic
      [] Doctors Office
      [] Hospital
      [] Other
      • Explain Other: [ANSWER IF NECESSARY]
WHY DO YOU WISH TO ESTABLISH THIS BUSINESS:
    • I wish to establish this business because it will not only benefit myself, but it will benefit Los Santos as a whole, since there is no longer any doctors available within All Saints General Hospital. I believe this business is going to be a safe place for members of our community to receive the medical care that they need. We have over five different specialities that make our clinic a one stop clinic. From our state of the art Radiology facility along with our forensic lab and our out-patient surgery.

    PLEASE STATE YOUR BUSINESS PLAN:
    • My business plan is very simple. I have already leased an amazing building with plenty of office space and had contractors in to build the facility as needed. We have ten members, five of which are the board of directors, one of those include me. There is no direct person in charge. We have five different specialties within our clinic which are the following: OB/GYN, Psychology, Cardiology, Forensic Medicine along with a General Surgeon. We plan on opening up the clinic to the general public to maintain a client base and provide the well deserved medical care to the community.

MEDICAL BUSINESS LICENSE REQUEST FORM

  • TITLE: Mr.
    NAME: Herbert Martin
    PHONE NUMBER: 555-4278-(2627)

    BUSINESS NAME:
    Los Santos Medical Research Laboratory
    TYPE OF BUSINESS:
    • [] Ambulance Service
      [] Medical Clinic
      [] Doctors Office
      [] Hospital
      [X] Other
      • Explain Other: A laboratory for the culture and application of human pluripotent stem cells (hPSCs) among other research into lesser known conditions/diseases such as: sudden infant death syndrome, carcinoma and fibrodysplasia ossificans progressiva. The laboratory will also act as a cancer research institute.
WHY DO YOU WISH TO ESTABLISH THIS BUSINESS:
    • To my knowledge, there has never been a facility in San Andreas that has risen to the challenge of researching and applying cell-based regenerative therapies. Proper planning of laboratory operations and proper design of the physical layout of a stem cell laboratory is a major undertaking that hasn't been done before. I have the drive, resources, time, and knowledge to devote my clinical practice to researching embryonic stem cells and their potential applications.

Currently, embryonic stem cells are being used to screen new drugs and in the generation of cells and tissues that could be used for cell-based therapies. Today, donated organs and tissues are often used to replace ailing or destroyed tissue, but the need for transplantable tissues and organs far outweighs the available supply. Stem cells, directed to differentiate into specific cell types, offer the possibility of a renewable source of replacement cells and tissues to treat diseases including macular degeneration, spinal cord injury, stroke, burns, heart disease, diabetes, osteoarthritis, and rheumatoid arthritis.

With the Los Santos Medical Research Laboratory, 'possibilities' can turn into a definitive 'able' in the world of stem cells. In 2018, an estimated 1,735,350 new cases of cancer will be diagnosed in the United States and 609,640 people will die from the disease. The Los Santos Medical Research Laboratory would like to join the fight in researching and looking for a cure to this deadly disease.

The Los Santos Medical Research Laboratory also plans to take a look at ailments, diseases, and problems that have been ignored by the medical community in the past. Sudden infant death syndrome (SIDS) takes the lives of over 3,600 babies every year and the cause still remains unknown. We're dedicated to all things research, and the 'unknown' doesn't deter us from picking up a topic and putting it under our lens.

    PLEASE STATE YOUR BUSINESS PLAN:
    • Establishing a well-functioning laboratory for the culture of human pluripotent stem cells (hPSCs) provides the foundation for successful culture and experimentation. With the special status of these cells, there is a higher degree of oversight, review, and reporting. We have found that a nearly equal amount of time and effort is required to establish initial protocols and seek approval for culturing and obtaining human pluripotent cell lines as is required to design and equip the laboratory.

As of current, there hasn't been any physician-scientists that have been employed under the Los Santos Medical Research Laboratory. Though, those who want to help the Los Santos Medical Research Laboratory don't need to be under our roof to do so. We plan to speak with other clinics in the area and ask for their aid and assign them a topic they may research on to add to our collective record.

The laboratory is still in the state of construction, but a building has been ordered and industrial-grade medical supplies have been bought from 'Armstrong Medical' among other medical retailers to run a standard hPSC culture laboratory. When the laboratory has finished being built, I predict innumerable growth.

The following list is our record of purchased equipment which will aid us in the basic culture and characterization of hPSCs:

      1.0. Tissue Culture Laboratory
      • Class II Biosafety Cabinet (BSC)
      • CO2 incubator.
      • Pipettors.
      • Vacuum flask/aspiration device.
      • Water bath (37°C).
      • Low-speed centrifuge (clinical grade, for spinning cells).
      1.1. Microscopy
      • Phase–contrast microscope.
      • Dissecting microscope.
      1.2. Storage
      • Cabinets and shelves for the storage of tissue culture supplies.
      • Refrigerator (4°C).
      • Freezer (−20°C, nondefrosting).
      • Low-temperature freezer (−70 to −85°C).
      • Cryogenic freezer (storage below −140°C, usually liquid nitrogen).
1.3. Molecular Biology Laboratory/Quality Control Laboratory
      • RT-PCR.
      • Flow cytometer (might be in a Core facility).
      • Fluorescence microscope (might be located in a Microscopy Core).
      • Confocal microscope (might be located in a Microscopy Core).
      1.4. Quarantine Laboratory
      • Class II Biosafety cabinet.
      • CO2 incubator
      • Phase–contrast microscope.
      • Water bath (37°C).
      • Low-speed centrifuge (clinical grade, for spinning cells).
      • Pipettors.
      • Aspiration/vacuum flask.
      • Sink.
1.5. Additional Access to Common Equipment or Core Facilities
      • Microscopy.
      • Flow cytometry.
      • Microarray gene expression.
      • Genomics.

  • TITLE: Mr.
    NAME: Larry G. Fischetti.

    Chetti&Asca Pharmacy
    TYPE OF BUSINESS:
    • [] Ambulance Service
      [] Medical Clinic
      [] Doctors Office
      [] Hospital
      [X] Other
      • Explain Other: Pharmacy (drug store).
WHY DO YOU WISH TO ESTABLISH THIS BUSINESS:
    • Right now in Los Santos, there are zero operating pharmacies which means doctors have to prescribe and hand out medicine directly to the patient. An operating pharmacy in the heart of Los Santos will aid many citizens, mainly the elderlies among us. Chetti&Asca Pharmacy will provide medicine and drugs to patients who obtained a prescription from licensed doctors. I believe a pharmacy in Los Santos is not just beneficial but necessary to the population in the city.

    PLEASE STATE YOUR BUSINESS PLAN:
    • Chetti&Asca Pharmacy will operate in order to meet the supply that is demanded by the population of Los Santos. The business will be opened every day throughout the week, handing non-prescribed medicine and prescribed medicine. Drugs and medicine will be obtained by licensed vendors and licensed pharmacists will be hired to work during the opening hours. Security will be hired as well due to the worth of the medicine inside the pharmacy.

A non-prescribed medicine, often referred an over-the-counter, or OTC, medicine-is any drug that you can buy without a doctor's prescription. Some common non-prescription medicines include:
      • Antacids and acid reducers.
      • Bulking agents, laxatives, and stool softeners.
      • Antidiarrheals.
      • Cold and allergy remedies.
      • Pain relievers.
      Some commonly prescribed medicines include:
      • Hydrocodone (combined with acetaminophen).
      • Generic Zocor (simvastatin), a cholesterol-lowering statin drug.
      • Lisinopril (brand names include Prinivil and Zestril), a blood pressure drug.
      • Generic Synthroid (levothyroxine sodium), synthetic thyroid hormone.

      [] Hospital
      [X] Other
      • Explain Other: Inpatient/Outpatient Mental Health Facility
WHY DO YOU WISH TO ESTABLISH THIS BUSINESS:
    • This business is being established as part of a larger effort by the Bureau of Social Services to provide mental wellness services. The facility features an inpatient wing for those in need of long term care as well as an outpatient wing for those who wish to attend counseling sessions. Individuals can sign themselves into the inpatient facility at any time, or they can be ordered by a court to sign themselves in - only a Social Worker can authorize the individual to leave the inpatient facility. This facility is being used to provide mental health services, both inpatient and outpatient, to those who are in need.

    PLEASE STATE YOUR BUSINESS PLAN:
    • We plan on operating this business as a direct division of the Bureau of Social Services, with all employees being Social Services Officials. Some employees will be designated Social Workers for the facility and will deal with the inpatient practice while others will be assigned to the outpatient practice to conduct regular group counseling and therapy sessions. As explained above, we intend on running the inpatient wing as an involuntary hold center while authorized by court order or the Mental Health Act. Individuals who are placed on an involuntary psychiatric hold or who wish to sign themselves into the inpatient practice will undergo group therapy sessions in a tightly controlled environment with 24/7 nursing staff and measures in place to prevent suicidality. The inpatient practice will dispense medications as needed to patients within the inpatient practice and will exercise the ability to chemically restrain any involuntary or otherwise violent patient by means of a combined shot of Ativan and Haldol. The doctor on staff will monitor all nursing and rehabilitation activities and will often issue prescriptions for various medications following a patient's discharge from the inpatient practice. Staff at the facility will keep a close eye on those admitted to the inpatient practice to ensure they are being treated.

      [] Doctors Office
      [] Hospital
      [] Other
      • Explain Other: [ANSWER IF NECESSARY]
WHY DO YOU WISH TO ESTABLISH THIS BUSINESS:
    • The main reason as to why I wish to establish this business is the fact that there are currently no medical clinics or enough hospitals / trauma centers open to provide the public with the medical treatment they deserve in a timely fashion. Our practice focuses on providing the best quality health care at minimum cost, making sure there are always doctors on call, whenever patients wish to schedule medical consultations or even if they just walk in.
Our incredible team of doctors and nurses wants to help keep Los Santos healthy, happy and sane. We have one goal in mind with this business and that is to help anyone with any support or medical needs.

    PLEASE STATE YOUR BUSINESS PLAN:
    • We’re currently situated on Central Street in Commerce, a busy market town located in the center of Los Santos, San Andreas.
If we get this license, our clinic will be able to offer a wide variety of services, such as vaccinations, sports injuries, health checks, emergency treatments, seasonal illness management and many others. We also intend to offer memberships to our Healthy Baby Club, a scheme that aims to make both pre-natal and post-natal care both safe and easy for parents and children alike.
In addition to the aforementioned, we wish to open up a partnership with the Los Santos Boxing Association, working to provide those who work within said field with the medical treatment they require and deserve after participating in straining combat matches.
We’ll be open on weekdays between 9am – 6.15pm and on weekends between 8:30am - 12:30pm, however our clinic will also offer a 24/7 out-of-hours service, managed by those who work at our practice. As a result, this means that we will be always available for our clients, regardless as to the time or day. Connected to our building is a large car parking area, with several bus stops located nearby as well, making our clinic an easy place to access for everyone.

    TYPE OF BUSINESS:
    • [] Ambulance Service
      [] Medical Clinic
      [] Doctors Office
      [] Hospital
      [X] Other
      • Explain Other: Community Center.
WHY DO YOU WISH TO ESTABLISH THIS BUSINESS:
    • Jefferson Community Center was set up with with the intention of providing low-cost health and community support to those in the locality. I have lived in Jefferson, L.S for some time now and having come from Ireland, I was shocked at the level of poverty and struggle on our city streets. The community center is not just dedicated to providing health services to those who struggle, but also to provide a range of services, all of which will be ensured success if the medical services were provided in-house. I hope to set up Alcoholics Anon & Narcotics Anon, as well as a family planning clinic offering birthing classes and parenting classes for low-cost, STI tests and I would love to introduce a counselling service to the center in due time.

There are no open-door policy spaces in Los Santos that I am aware of, and certainly none in the east of L.S where gun-crime is at an all time high. In the future, I hope to work with the many new and up-coming health-care providers across Los Santos in order to provide a complete and beneficial support network for the community and our cities general population.

I established this business because I want to make a difference to people's lives and inspire change, and improvement.

    PLEASE STATE YOUR BUSINESS PLAN:
    • I have obtained a business lease for a central location on Santa Rosalia Street, Jefferson. I have been working hard to renovate the location, and provide my clients with a spacious and sanitary social space. This includes two office spaces, a large social space containing our computer corner.

We have a range of non-medical services which include free internet access, homework assistance, translating services, citizen advice however I wish to exceed all expectations at the center and hope to incorporate more and more medical services as time passes (and my training progresses).

Within five years, I hope to have an in-house therapist/psychologist, G.P/M.D, and other health-care professionals working daily within the center. These medically trained personel will allow for our services to be properly implemented, and allow for our clients to be well supported by trained and experienced staff. I personally wish to set up a community outreach scheme, providing the public (local youth in particular) with life-saving techniques and other basic first-aid. I would love to hold informational evenings, and self-defence classes within the center's large social space. I would not feel comfortable inviting the public to the community center without our staff being adequately trained in the case of any emergency/accident occurs.

I have also been working closely with a member of LSPD, and have many youth rehabilitation programs in mind, again, having available medical staff on-hand will be a great aid to ensuring this service is successful.

I also intend to open Monday to Saturday, 2.30pm - 6pm. JCC will have emergency contact information available, as like I said previously we have an open-door policy and will do anything within my power to ensure my clients/community members are cared for. Our location has decent foot-fall, and a small parking lot outside, wheel-chair access, and is conveniently located on the local bus route.
